Note: Proof of completion of the 2 dose Moderna COVID-19 Vaccination is required before receiving your booster dose. Please bring a copy of your vaccination card to your scheduled appointment, or we will not be able to administer a booster dose.

The Moderna COVID-19 Booster Vaccine (3rd dose) are being recommended for individuals: 

• 65 years of age or older
• Living in long-term care facilities
• 18-64 years of age with underlining medical conditions
• Where it has been at least 6 months after completion of initial two-dose series
